Project Access encourages you to complete the application online and submit. If the application is printed out, please fax, mail or email to Project Access. The steps below outline what you can expect in the application process. If you are not an eligible applicant, you will be directed to the appropriate program based on the information you have submitted.
Submit all required documents with application by fax, mail or email to Project Access.
Once your application and corresponding required documents are received, the application will be reviewed and processed within 24 hours.
Project Access will contact each applicant with eligibility results once the application is reviewed.
If the applicant meets the eligibility requirements for enrollment, Project Access will schedule an appointment.
